Responsibility for the Hospitalist (Exempt) Job

  • Deliver clinical care to acutely ill patients in the HaH service, conducting daily visits either virtually or in person based on patient needs and circumstances.
  • Provide acute-level care as a crucial member of the Mercy HaH team, including clinical documentation, treatment planning, medication management, and medical oversight.
  • Offer medical direction oversight to HaH clinical field staff and interdisciplinary teams to ensure high-quality clinical care and outcomes.
  • Supervise and oversee dedicated Mercy HaH Advanced Practice Providers (APPs) as a Collaborating Physician.
  • Conduct daily patient care visits, and complete all necessary clinical documentation, orders, and chart reviews following HaH policies and procedures.
  • Collaborate with a multidisciplinary care team to identify patients meeting HaH criteria and provide clinical care to acutely ill patients.
  • Coordinate in-home diagnostics, point-of-care testing, medication administration, and patient monitoring, and address any other patient needs as they arise.
  • Participate in quality measurement and improvement initiatives to advance the HaH care model and other alternative care pathways.
  • Work on a competitive, shift-based model (7on-7off).
